Auto & Workers’ Compensation:

We accept all auto and workers’ compensation claims, but patients must provide a claim number and adjuster contact information before the appointment in order to be seen.

Healthshare Plans:

We treat Healthshare plans as self-pay. Patients are required to pay the time-of-service fee upfront. A receipt will be provided for you to submit for reimbursement through your Healthshare provider.
